You can use the form below to request non-circulating materials from Cudahy Library, Lewis Library, and the Library Storage Facility. We will hold any materials that you request at the Cudahy Library Circulation Desk for 10 days.

Before placing your request, please consider the following information:

• Materials on Course Reserves are not available for request. To borrow these materials, please visit the Circulation Desk at the appropriate library.
